Office Policies

Insurance information:

Patients are required to bring in their current insurance information to their appointments, if we do not have the correct insurance information on file you may be held responsible for the visit. We accept the following insurance plans: AARP, Aetna, BC/BS, Cigna, GHI, Independent Health, Medicare, Medicaid, Nova, Tricare, United Health Care and Univera.

Co-payments:

Co-pays are due at your appointment. Each patient is responsible for knowing his or her plan’s benefits package, co-payment, co-insurance, deductible, non-covered services, and restrictions. We cannot quote coverage or benefits. If you have questions regarding coverage or benefits, please call your insurance plan.

Appointments:

Our office requires 24 hour notice if you need to cancel or change your appointment. Should you not provide this notice to the office, a $25 no show or late cancellation fee may be added to your account.

Referrals

If your insurance plan requires a referral please contact your primary care doctor to have them process this before your appointment. If you do not have your referral, we may ask you to reschedule your appointment.
